SoonerCare uses the 2025 Federal Poverty Level (FPL) guidelines. These took effect April 1, 2025. They remain active through March 31, 2026.

  • One person: $15,650 per year ($1,304 monthly)
  • Two people: $21,150 per year ($1,763 monthly)
  • Each added person: +$5,500 per year

Adults 19 to 64 (Expansion Group)

Adults without children can qualify. Oklahoma expanded Medicaid in 2021 via State Question 802.

Pregnant Women

Parents and Caretakers

This group has stricter limits. Many now qualify under the expansion group instead.

Income Limit: 41-100% FPL ($535–$1,304 monthly)

Aged, Blind, and Disabled (ABD)

These applicants use Non-MAGI rules. Asset tests apply.

How Does SoonerCare Count Income?

Most applicants use MAGI. This stands for Modified Adjusted Gross Income. It matches your tax return data.

MAGI Groups:

What Income Counts?

MAGI counts wages and self-employment. It counts unemployment and pensions. It counts interest and alimony.

MAGI does not count child support. It does not count SSI or SNAP. It does not count Native American payments.

The 5% Disregard

MAGI rules allow a 5% income disregard. If you earn slightly over the limit, you may still qualify.

2026 Policy Updates

SoonerCare has new rules for 2026. These changes help families keep coverage.

Postpartum Coverage: 12 months of coverage after birth is now permanent. This became law under the 2023 Consolidated Appropriations Act. It applies whether the pregnancy ends in birth or miscarriage.

Kids’ Coverage: Children keep coverage for 12 months straight. Income changes during the year do not matter.

Provider Changes: Community Health Worker services expand on April 1, 2026. Some prior authorization rules eased February 1, 2026.

Work Requirements: New rules may require work for some adults. These follow the One Big Beautiful Bill Act of July 2025.
